Health Forms
Thank you for helping us keep our community safe. Your child's health is important to us.
For each required document, please complete the form in its entirety. Forms that are missing the physician's name, signature, or complete medication instructions will not be accepted. All forms must be submitted directly to the school nurse and should not be dropped off elsewhere.
Medications must be:
- Unopened
- Properly labeled with the child's name and date of birth
- In the original prescription container with the pharmacy label attached

If Your Child Needs Medication at School
The following documents are required:
- Food Allergy & Anaphylaxis Emergency Care Plan
- A Consent to Administer Medication form
- Asthma Action Plan
- Dietary Accommodations Form
Both forms require the physician's signature.
Please take all required forms to your child's healthcare provider to be completed and signed.
Example: If your child has asthma, you must submit both the Asthma Action Plan and the Consent to Administer Medication form.
